FEES, RATES, & YOUR FINANCIAL COMITTMENT TO YOUR HEALTH
STANDARD RATE. Therapist rates are $100/hour ($125 for initial appt). Reduced rates for hardship cases can be addressed with your assigned therapist, and approved the practice manager. INSURANCE PLANS. You may use your health plan if your assigned therapist accepts your plan. Accepted plans are listed under each therapist’s biography on the therapist page. Your financial obligation (copayments) based on your health plan will be emailed to you prior to your first appointment.

FINANCIAL COPAYMENTS. Your copayments are due at the time of your appointment, and collected remotely by our Billing Dept. Your fees are charged to your credit card within 24 hours of your appointment.
DOCUMENT REQUESTS. You are charged $50 per document for requests for documentation sent directly to attorneys, or other outside agencies. There is NO CHARGE for documents sent directly you. Documents sent directly to you will be ONLY sent electronically through your Secure Online Portal Account.
PAYMENT AUTHORIZATION & FINANCIAL CONSENT
Clients must maintain a current credit/debit card on secure file with us through your Secure Portal Account. Clients must also electronically sign a Payment Consent Form for the current credit/debit card on file. Appointments must be suspended if payment cannot be collected by the Psyched Group.
GETTING INITIAL APPOINTMENT
A PsychED therapist will assess your situation prior to your first appointment. Therapists do NOT provide free phone consults prior to appointment but our Intake Coordinator can provide 15 minute consults. In order to REQUEST an initial appointment with a therapist you may setup a secure online account, fill out the self-assessment questionnaire, and request an appointment with an available therapist. That therapist will assess your situation by reading your self-questionnaire. If a therapist does not visit with you after reviewing your self-assessment, then your portal account and ALL information on file will be permanently deleted for security purposes. We will do our best to offer referrals to other providers that better meet your needs.
CANCELLATION POLICY
If you do not show for appointment or cancels within 24 hours of the appointment then a $75 “Late Cancel/No Show” Fee is charged. Appointments cancelled before 24 hours of appointment will not be charged because your therapist is able to schedule another client in need. Your therapist may submit a waiver to reduce this fee in cases of hardship.

We are committed to respecting and protecting your privacy and the confidentiality of your health care information. Protecting your privacy and the confidentially of your medical information is a critical factor in providing mental health services. The code of ethics HIPAA (Health Insurance Portability and Accountability Act) as well as CHAPTER 42 CFR Part 2 (Confidentiality of Alcohol and Drug Abuse Patient Records) mandates that all information about you be protected and that any disclosure of your protected Health Information (PHI) requires your written consent.
